Hi im posting this on behalf of my brother ckorus, who doesn't have a forum account.
We recently went to the nether and he got kill by zombie pigmen. When he got back his stuff wasn't there anymore. Not to mention there wasnt a [Clear Lag] thing. The main stuff he lost was: Diamond efficiency 3 unbreaking 3 pick and a diamond fortune 3 efficiency 4 unbreaking 3 pick. The pigman probably stole his items.

A pigman probably stole his stuff, he waited to long to go get it OR there was a clear lag wipe. Sorry but we can't replace the stuff.
